A hack-a-thon with two primary focuses: loneliness and isolation, and dementia.

This hackathon will bring together older members of our communities alongside older people, befriending and dementia services to discuss what it means to be an older person in our communities and what challenges they face on a daily basis.

Come along if you:

  • Would like to create solutions with professionals and people who use services, carers and parents
  • Would like to use your own experiences (both professional or lived) to influence services for older people and people living with dementia

As part of the hack-a-thon, we will work in small groups to look at people’s stories and examples, and co-design potential solutions for improvement.

Each group will be led by a creative individual who will support in facilitating meaningful conversations and a creative piece, for example a song.

The workshops we will cover include:

Community based support; loneliness and isolation; good dementia care; home from hospital; independent living

Accessibility

The event venue is fully accessible.  Please let us know if you have accessibility needs when you register.

Refreshments

Refreshments and lunch will be served throughout the day.

Parking

On-site parking is available with allocated disabled bays.

Public transport

The venue is within walking distance of Treforest train station and bus stops.

We are able to fund transport for those who are attending – individuals and groups are kindly asked to co-ordinate their own transport, and we can then provide financial support on receipt of an invoice.

Pobl Hyn a Dementia

*Cofrestrwch drwy'r ddolen ar waelod y dudalen hon*.  Cysylltwch â ni os oes gennych unrhyw broblemau. Diolch.

Hac-a-thon â dau brif ffocws: unigrwydd ac ynysrwydd, a dementia.

Bydd yr hac-a-thon hwn yn dod ag aelodau hyn ein cymunedau ynghyd ochr yn ochr â gwasanaethau i bobl hyn, cyfeillio a gwasanaethau dementia i drafod ystyr bod yn rhywun hyn yn ein cymunedau a pha heriau a wynebir ganddynt yn feunyddiol.

Dewch heibio os ydych chi:

  •  Eisiau creu atebion gyda gweithwyr proffesiynol a phobl sy’n defnyddio gwasanaethau, gofalwyr a rhieni
  • Eisiau defnyddio eich profiadau eich hunan (boed broffesiynol neu o fywyd) i ddylanwadu ar wasanaethau i bobl hyn a phobl sy’n byw gyda dementia

Fel rhan o’r hac-a-thon, byddwn ni’n gweithio mewn grwpiau bach i edrych ar straeon ac enghreifftiau pobl, a chydgynhyrchu atebion posib er mwyn gwella.

Arweinir pob grwp gan unigolyn creadigol a fydd yn cefnogi hwyluso sgyrsiau ystyrlon a darn creadigol – cân, er enghraifft.

Bydd y gweithdai y byddwn ni’n ymdrin â nhw’n cynnwys:

Cefnogaeth a seilir yn y gymuned; unigrwydd ac ynysrwydd; gofal dementia da; dod adref o’r ysbyty; byw’n annibynnol

Hygyrchedd

Mae lleoliad y digwyddiad yn hollol hygyrch. Gadewch i ni wybod os oes gennych anghenion hygyrchedd wrth i chi gofrestru.

Lluniaeth

Gweinir lluniaeth a chinio drwy’r dydd.

Parcio

Mae parcio ar gael ar y safle gyda mannau anabl wedi’u clustnodi

Trafnidiaeth gyhoeddus

Mae’r lleoliad hwn o fewn pellter cerdded i orsaf fysiau Trefforest a mannau aros bws.

Rydyn ni'n gallu ariannu trafnidiaeth i'r rhai sy'n mynychu - gofynnir yn garedig i unigolion a grwpiau gydlynu eu trafnidiaeth eu hunain, ac yna gallwn roi cymorth ariannol ar dderbyn anfoneb.
